Register with our practice

We are currently accepting new patients, to register as a patient please check you are within our boundary (please see below).

To register with the practice, you need to complete a registration for each patient by clicking on the link below.

Alternatively, you can come into the surgery and ask for a registration form. Before your registration can be accepted you will need to present 2 forms of identification to reception. One to confirm your personal identity (e.g., passport) and one to confirm your address (e.g., utility bill).

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

Named GP

All patients will be allocated a named accountable GP.

The role of the named GP is to take responsibility for the co-ordination of all appropriate services and ensure they are delivered where required (based on the named GP’s clinical judgement) to each of their patients. The named GP will deal with all correspondence received from hospitals and other organisations regarding your care, and will review any repeat medication you have.

Having a “named GP” does not mean that this is the only doctor or clinician at the practice who will provide you with care. You do not have to see your named GP you can continue to feel free to choose to see any GP. If you need to be seen urgently this may be with any of the doctors working that day.

If you have a preference for your named GP please let us know and we will make every effort to accommodate this request.

Access to health records

Patients over 16 are automatically given online access to their records from the date of registration and will be able to order repeat medication online. For patients who are unable to see their records online or require full access, please download and complete an online access form below and bring it into the surgery with 2 forms of identification, one of which will need to be photographic ID.

Evidence shows that patients who are better informed and involved with their own care have better health outcomes and are less likely to be admitted to hospital.

Patient Online/Proxy Access is now available to:
• Renew Prescriptions online
• View your GP record online

To apply for Online/Proxy access we will require:
• An up-to-date e-mail address
• The completion of our standard Online Access form (this is available from reception or upon request)
• Suitable identification (see below)

To apply for Online Proxy Access for Carers or Parents/Guardians for children under the age of 11 yrs we will require:
• A Proxy Online Access form to be completed (this is available from reception or upon request)
• Suitable identification (see below)
Please note, for children between 11ys and 16yrs Proxy Access is subject to capacity or with the child’s written permission

ID required to set up Online/Proxy Access

• For access to a child’s record under 11 – birth certificate of child & photographic ID of parent/guardian.
• For access to your own record – two forms of identification, one of which must be photographic
• For access to someone you care for – photographic ID for you and person you care for

For access to the records of someone lacking capacity/unable to consent:
• Your own photographic ID
• A copy of the registered power of attorney (Health and Welfare) for the person lacking capacity.
• A copy of the Court Appointed Deputy documents
